Summary
We are offering the exciting opportunity for 5 Social Care Practitioners to join our Ongoing Support Teams based in Trowbridge, Chippenham, and Salisbury, and 3 to join our countywide Reviewing Team.
LIVE WORK SUPPORT
Our Ongoing Support Teams are locality based and offers social work and occupational therapy services to those who require longer term social care support.
We collaborate with individuals and communities to promote resilience, independence and wellbeing.
Our new Reviewing Team will work across Wiltshire to complete Direct payments, scheduled/annual and out of county reviews, on behalf of the Ongoing Support Team. This team will be in place initially 12 months in order to manage backlog of reviews awaiting completion.
About the role
As a Social Care Practitioner, you will provide person centred and outcome focused assessments and reviews to identify the support required to enable customers to make positive changes in their lives.
You will promote customer independence, choice, and well-being in the community, using a strength-based approach and utilising universal services and support where appropriate.
This is a challenging role, and you will be supported by a Team Manager, an experienced Social Workers and highly experienced peer Social Care practitioners.
About You
You should hold a level 4 in Health and Social Care, equivalent or the motivation to work towards this qualification, as well as experience of working in a health or social care setting.
The work is fast-paced, and you should have excellent communication skills, and the ability to work collaboratively with multi professional teams.
IT skills are also essential, and experience of Liquid Logic would be beneficial.
