Full Job Description

  • You will be responsible for direct care delivery for people with a diagnosis of organic mental health illness and associated problems related to the Aging process, in particular, Dementia, Challenging Behaviors’ and Physical Disabilities.
  • The Walton unit provides an assessment and treatment service for older adults with the organic illness. There is a multi-professional team working at supporting the needs of the patients with the aim of fostering hope and quality of life,
  • You will carry out all relevant forms of care in support of and supervised/directed by a Registered Nurse and will support the Registered Nurses in the implementation and delivery of high standards of patient care promoting patient well-being following a ‘Person-Centred Approach
  • Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
  • We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centers, as well as in clinics, GP practices, schools, care homes, and, increasingly, in people’s own homes and via virtual consultations
  • During the pandemic response, we have led the implementation of public vaccination centers in collaboration with partners across health and social care.

  • To support registered nurses in the implementation of a high standard of patient care promoting patient well-being following a ‘Person-Centred Approach.
  • Follow nationally accepted guidelines as directed when dealing with aggressive/abusive situations and attend the Managing Violence and Aggression courses as mandated whilst supporting patients, staff members, carers, and the public in stressful situations
  • Use judgment to identify challenging situations and defuse and distract patients to promote the safety of patients, other staff members, carers or other visitors to the ward.
  • Report any concerns or problems to supervise nurse, particularly relating to individual patient safety.
  • Support patient care needs in generic health resources both inpatient stays and out-patient appointments as directed
  • The post holder will participate as a member of the nursing team in the delivery and monitoring of individual care/treatment plans and report changes to the registered nurse as appropriate.
  • undertake programs of personalized care to meet the direct care needs of each patient, maximizing independence and “recovery”
  • Report and document any concerns or abnormal changes in the patient’s physical or mental well-being to a registered nurse.
  • Be aware of and follow general care routine within the area, with particular reference to patients who present a high risk.
  • participate in social and therapeutic programs of care with patients to achieve a meaningful and stimulating environment.
  • Ensure patients’ meals are prepared and served in an attractive manner, taking into account personal choices, likes and dislikes, ensuring a balanced diet is offere
  • Supporting and helping patients maintain health and well-being.
  • Maintain intensive support of patients who are exhibiting periods of aggressive behaviors, ensuring that specified risk factors are acknowledge
  • Support formal and informal patients, following treatment plans, Section 17 Leave arrangements and risk assessments across hospital and community health-based settings.
  • Assist patients in meeting social, leisure, and spiritual needs escorting patients as directed by the registered nurse in the local community taking into account any emerging risk requirement remedial actions.
  • Ensure that all patients are afforded due respect, dignity, and privacy.
  • Maintain observation requirements of individual patients as directed on an intermittent or continual basis in line with NICE guidance.
  • Display high levels of effective interpersonal skills using both verbal and non-verbal skills to support patients in highly emotional states and patients who cannot communicate due to poor cognitive condition/memory loss.
  • Record care interventions, EWS, results in core records, and report findings to registered staff.
