Job Description:

This is a full-time, permanent, classified position which is represented by a union.

The

Oregon Health Authority (OHA), Oregon State Hospital (OSH)

has a career opportunity for a Pharmacist in Salem, Oregon to work collaboratively with providers to provide medical treatment to mental health patients in a hospital setting.

The Oregon State Hospital provides patient-centered, psychiatric treatment for adults from throughout the state who need hospital-level care. Our primary goal is to help people recover from their illness and return to the community. You will work in a secure, seven-day-a-week facility and may work holidays and weekends if assigned. Daily onsite presence is necessary though occasional telework may be approved. You may be required to work hours that are subject to change with little notice, and/or work hours that exceed regular schedules. You may be required to carry on-call phone daily from 7:00 AM to 6:00 PM during weekly assigned shifts.

What will you do?

As a Pharmacist, you will perform the following duties:

  • Work collaboratively with a team of pharmacists, pharmacist informaticists, and inventory specialists to optimize medication distribution from the central pharmacy to 24 patient care units.
  • Provide professional supervision of pharmacy technicians.
  • Perform prospective drug utilization reviews and transcription into Avatar/RxConnect platforms.
  • Support clinical services including anticoagulation management and direct patient care, clozapine and diabetes management, antibiotic stewardship, and transition of care review and reconciliation.
  • Serve as an important resource for other healthcare professionals in accordance with applicable Federal and State Laws, and Joint Commission accreditation standards.
  • Ensure that pharmaceutical and related services are always available on both OSH campuses.
  • Provide medication information and education related to mechanism, utilization, administration, side-effect management, and monitoring to practitioners, nursing staff, or patients.
  • Participate in quality-assurance activities, including medication error reporting.
  • Review and process outpatient medication orders in accordance with regulations, best-practice standards, and OSH policies
  • Perform pharmacist on-call responsibilities by remotely monitor medication order submissions and triage to review and transcribe all after-hours medication orders in a timely and appropriate manner.

What are we looking for?

Minimum Requirements

A current Pharmacist’s license issued by the Oregon State Board of Pharmacy.

Special Qualifications

Must be a graduate of an accredited school of pharmacy with a PharmD and be a registered member in good standing with the Oregon Board of Pharmacy.

Requested Skills

  • Preference will be given to applicants who have two or more years of experience working in a behavioral health and/or long-term care clinical practice setting.
  • Completed a PGY1 pharmacy residency and/or PGY2 psychiatric residency, and/or possess BCPP, BCPS, or BCACP board certification.
  • At least 3 years’ experience in an inpatient hospital pharmacy setting.
  • Experience working with mental health patients and treatment.
  • Experience precepting pharmacy students/residents.
  • Knowledge of Omnicell, EHR functionality.
  • Must have a valid driver’s license or the ability to provide an alternate means of transportation to travel between Salem and Junction City campuses.
  • Must be able to flex hours or work additional shifts when necessary to complete time sensitive projects or ensure the safe and efficient operation of the facility.
